A simple “patient” observation chart for parents

Having three young kids, Susie and I are used to having to regularly take their temperature when they’re ill, give them Calpol or Nurofen when their temperature’s high, and make a note of it so that we both know what’s going on.  However, these notes usually end up on the backs of envelopes and so on and it’s easy to lose track of where we are.  I’ve googled a couple of times for a simple observation chart/sheet that we could use, but mostly I’ve found over-complicated hospital PRFs and the like.  So I’ve rolled my own.  It’s pretty basic, but that’s the idea.  You can download it from here and use it as you wish.  The “more sheets” part is to be filled in last, and indicates that there’s either another sheet that’s been started or that the illness has passed and that’s the last sheet.  I imagine the cases where Y is circled will be more common (and useful) than those where N is circled, but you get the point.

Anyway, I hope you find it useful.  If you do then please do leave a comment.  It’d be good to know that I’ve made something useful for people!
